Animal Coalition of Delaware County would like to thank you for adopting a homeless animal. We hope the materials and information you received at the time of adoption have been helpful while your new compantion settles into your home.

In an effort to get an idea of how well your new family member is working out, we ask that you complete this post-adoption follow-up survey so that we might be better able to help you.  Please note that you do not need to register on our website in order to complete the form; however we do use a verification code system in order to be sure that a real live person is filling this out!

You will see a message that says "Before submitting this online form, we want to verify that you are a human, and not an automated system trying to flood us with spam." There will be a small box where you will be asked to enter a verification code that is displayed on the screen. If you do not see a code on your screen, click your mouse inside the box and one should then appear onscreen. Then you can type the code in, then click the "Verify" button. You may then proceed with the survey.
